These forms record a practical induction with an experienced member, using the actual machine and its manual. They supplement the manual. Submitting a form does not itself authorise solo use.

Using these checklists

  1. Choose the correct tool and power type above.
  2. Work through the checks and practice with an experienced member. Complete the form yourself, marking each point I understand and have covered this, N/A or I need more help.
  3. Record your agreed task, limits and outcome, then enter your own name and date as a typed sign-off.
  4. Enter your email address carefully and submit. Google Forms sends you a copy of your answers, saves the record in RVAS’s private Google Drive and sends a notification to rvascontact@gmail.com for the RVAS team to review.

If you complete a paper checklist, keep it and enter your answers in the matching online form when you want an emailed copy and a record saved in RVAS Drive.

If more help is needed, no solo use until the outstanding points have been addressed and recorded. These forms are induction records, not booking forms or confirmation that a particular machine is available.

The experienced member must know how to use the machine safely and explain it clearly. Allow time for questions and practice. They should watch you checking, handling, starting, using and stopping the machine. Explain any N/A answers and record the limits you agree together.

Emergency arrangements

Complete and explain the first-aid kit location, site address or postcode and emergency entrance or access in the form during the induction. If someone is injured, stop and make the area safe without putting yourself at risk. Get first aid; call 999 or 112 for serious injury and arrange access for responders. Report accidents and near misses to the Tool Librarian.

Keeping records and refreshing skills

Keep your emailed copy. RVAS keeps the submitted record with its tool induction records; individual answers are not displayed on this website. Revisit the induction after a relevant change, incident, concern or a long gap in use. Before every use, check the machine and working area and stop if unsure.

For help, a missing copy or a correction, contact the Tool Librarian at rvascontact@gmail.com.
